Practice Policies Introduction This document is intended to provide important information to you regarding your treatment. Please read the entire document carefully and be sure to ask me any questions or share any concerns that you may have regarding its contents before signing it. It is your right to have a complete explanation for any questions you may have, now or in the future. Although I know this may be uncomfortable at times, your openness and honesty will allow me to better serve you. About Me: I am a Licensed Clinical Social worker with a license in both Illinois and Florida. I hold a Master’s Degree in Social Work from Aurora University, Illinois and a Master’s Degree in Educational Administration and Supervision from Roosevelt University, Illinois. I utilize a variety of interventions based on my client’s needs, including CBT, DBT, Psychodynamic, Person-Centered, Strength Based, Inner Child Work and Meditation. I have worked in local and federal government agencies as well as private practice. I adherer to the NASW Code of Ethics. I work with adults experiencing anxiety, depression, PTSD, and trauma (religious and sexual) in my practice. Appointments, Scheduling, & Cancellations: You may schedule a session with me by phone, text or email. Your initial session will consist of an evaluation including reviewing history, presenting concerns, current symptoms, and treatment goals. Scheduling future sessions will depend upon whether your needs can be adequately and ethically addressed. If your needs cannot be addressed by this provider you will be referred to other providers. Appointments are usually scheduled on a weekly or bi-weekly basis and standard individual therapy appointments are 55-60 minutes in length; additional time may be scheduled as needed with therapist ahead of time for an additional charge. If you are late for your appointment, your session will not be extended and will still end at its normal time. Please remember to cancel or reschedule at least 24 hours in advance. Cancellations, rescheduling and no-shows are subject to an $80.00 charge if not received at least 24 hours in advance, and will be charged to the credit card on file. This is necessary because a time commitment is made to you and is held exclusively for you. By signing this consent document you are authorizing The Conscious Awakening Practice, PLLC to charge your card on file for a cancellation or no-show per the above policies.
